1.1. All personal data I provide in the application, both my own and those related to
patient measurements, are truthful, accurate, and have been collected lawfully.

• ▪ Enter cranial measurements to calculate relevant indices for cranial deformities (Cranial Index and Cranial Vault Asymmetry Index).
• ▪ Monitor the evolution of cranial measurements through graphs (premium feature). Provide recommendations to caregivers based on the infant’s condition.

1.2. I acknowledge that I am solely responsible for the accuracy and legitimacy of the data entered in the application and that such data will be used exclusively within the framework of the functionalities offered by Headshape.

2.1. I commit to maintaining the confidentiality of the patient data recorded in the app and to complying with all applicable legal regulations in my jurisdiction, including those related to personal data protection, particularly Regulation (EU) 2016/679. I declare that I am solely responsible for the patient data I upload to the application. including images, measurements, and any other related information.

2.2. I ensure that I have obtained the explicit consent of the patients or their legal representatives to collect and record measurements in the app, as applicable. Specifically, I declare that I have obtained the express consent of parents or legal representatives for collecting the data of minors under 16 years old or individuals with diminished legal capacity and that I have duly informed them of their rights concerning the data in accordance with Regulation (EU) 2016/679.

2.3. No personal user data will be shared with third parties without explicit consent. For more information, please refer to our [User Data Privacy Policy].

3.1. I declare that I will use the application exclusively for professional purposes related to my declared role in the user registration form and that I am aware of and commit to respecting the code of ethics applicable to my profession as established by the relevant Professional Association.

3.2. I acknowledge that Headshape is a professional support tool and that its results do not replace my clinical judgment or medical decisions based on my experience and knowledge.

4.1. I accept that the Headshape development team is not responsible for the accuracy of the data entered by me nor for any interpretations or decisions I make based on the results generated by the app.

5.1. I expressly authorize the developers of the Headshape application to process and store the data entered in the app solely for the purposes established in the terms and conditions of service and with respect to patient data under the agreed pseudonymization conditions.

5.2. I declare that I have read and understood the Headshape Privacy Policy and accept its terms.
